Child Support Guidelines                               GET AFFIDAVIT FORM HERE 

Please return the original affidavit, only once notarized, along with the following: 

• A copy of your VALID STATE DRIVER’S LICENSE OR STATE ID (if mailing in, please make sure it is a clear, legible copy of the photo).
• If remarried, a copy of your marriage license.
• If returned to maiden name, a copy of your divorce decree.
• If you have moved and failed to change your address with this office, we will require something with your old address as well as new (i.e. utility bill, tax bill)
